I/We confirm the firm:

• is registered with the Solicitors' Regulation Authority and adheres to the SRA Financial Services (Conduct of Business) Rules 2001 (and as amended)

In relation to insurance related activities the firm is either:

• authorised and regulated by the Financial Conduct Authority (FCA), or
• noted on the FCA register as an Exempt Professional Firm and have appointed an Insurance Mediation Officer

I/We will notify Wesleyan of any change of this status.

I/We confirm that we will pay premiums for all policies bound for clients to Wesleyan within 30 days of the cover date or date of account statement (as applicable).
